So we can follow up if we have questions — we will never share your email.

Something simple is fine. Even just "My Story" works.

Write as much or as little as you'd like. Survivors, caregivers, families, and supporters are all welcome here.

How Would You Like Your Story to Be Shared?
Publicly, with my name. Your story may be featured on our website, blog, or social media with your name attached.
Publicly, but anonymously. Your story may be shared publicly but your name will never appear — we'll say "shared by a community member" or similar.
Privately — with the Foundation only. Your story will be read by our team but never shared publicly in any form. It still matters to us.
